Format requirements for physical examination form 

    Please note that the candidates taking the dual-certificate MPA entrance re-examination 2012 will personally go to an upper second-class hospital or above for a physical examination, please download the following Physical Examination Form for Postgraduates, which shall be filled in with the physical examination results and bear a hospital seal.
